POWER OF ATTORNEY
A Medical Power of Attorney appoints a specific individual to make decisions on your behalf should you be unable to make decisions yourself. These decisions relate to the health of an individual and the care that should be administered if necessary. The Medical Power of Attorney also allows an individual access to medical records and health information in order to make appropriate and necessary decisions.
Limitations on the Medical Power of Attorney may be put into place to provide special directives under extenuating circumstances.
